PUPPY PURCHASE AGREEMENT/GUARANTEE

The Following agreement is between Cynthia Johnson, hereafter referred to as "seller"
and                                             hereafter referred to as "buyer" for the sale and purchase of
one purebred dog of the following breed, Pug or Bulldog on date                    . Purchase
price is                    .
Seller certifies that this puppy is a purebred dog of a sound mind and body, in good health to
the best of my knowledge, and free of communicable disease as it appears to the eye.
This puppy has been checked by a Veterinarian and was found to be healthy. This puppy
has received one set of shots and has been treated for worms. This puppy has also received
a ten day treatment of Albon.
Buyer has forty eight hours from date of purchase in which to have puppy checked by a
licensed Veterinarian, and is urged to do so. In the event that the date of purchase is on a
holiday or other time when the Veterinarian's office would normally be closed, then this
guarantee shall be extended to the next business day. Failure to do so will void any and all
warranties or guarantees. If the Veterinarian finds anything seriously wrong with the puppy
(limited to congenital birth defect causing death, or communicable disease acquired before
leaving my premises), Upon receipt of a letter from the examining Veterinarian (offered as
evidence of the puppies illness), the puppy will be exchanged within six months of the original
date of purchase with a puppy of the same kind, or the purchase price will be refunded.
Puppy must be returned to me within the guarantee period. No exchanges will be made after
the guarantee period.
I also offer an extended six month guarantee on genetic defects that would cause death.
Puppy will be replaced with another puppy within six months.
Please note: Because of their solid foundation of good health, good food,prior vaccinations,
and lack of exposure, it is unlikely that our puppies will contract a contagious disease if you
follow the recommended vaccinations and provide proper Veterinarian care. Please use
common sense and do not expose your puppy to other dogs or public places until he/she has
had a series of vaccinations as recommended by your Vet.

Seller is not responsible for the puppies size, color, or behavior at maturity. The seller
assumes no responsibility for the puppy after leaving the premises, including, medical or
veterinarian expenses, mortality, or any other reason except congenital birth defect causing
death (requires a necropsy report from the examining Veterinarian and registration papers).

Please note: Ear mites, worms, coccidian, or parasites are not serious health problems if
properly treated, and are not covered by this warranty.

              SPAY/NEUTER AGREEMENT
This puppy is being sold and warranted for the purpose of being a pet/companion only.
Spaying or neutering is required before the puppy is six months old(when applicable). This
will eliminate certain health issues that could be deadly to your puppy, such as cancer in both
male and females. It will help avoid undesirable traits such as marking territory in males and
heat cycles in females. It will also prevent an accidental breeding and an unwanted litter of
puppies.

Buyer agrees to provide excellent care and a loving environment for this puppy. Buyer also
agrees to house the puppy primarily as a member of the family. Please be aware that due to
the facial structure of pugs and bulldogs, they can overheat quickly and as a result can die of
heatstroke.

Seller will gladly accept return of the puppy , at any age on a rescue basis, if buyer is ever
unable to keep it.
This warranty is nontransferable, and is void if the puppy is sold and/or full or partial
ownership is transferred to a third party.
Registration papers will be furnished to buyer when complete payment is made for the puppy.
